Health Questionnaire

Yoga practice includes breathing techniques, meditation, relaxation and physical activity involving weight bearing. Instruction is designed to minimise risks. If you feel pain, discomfort or unwell at any time, please stop, rest and communicate with the teacher.

Certain health conditions require special attention. If you have a health condition or injury, please obtain clearance from your GP or other medical professional before commencing class, and inform the teacher of this. Certain yoga practices are not suitable for mental health conditions. Please disclose any such conditions in this form.
